Work Environment
This position requires extensive travel across East Brunswick and surrounding areas, making reliable transportation essential. Physical Therapists will perform assessments and treatments in a variety of home environments, enabling them to develop meaningful relationships with patients in their own spaces. This onsite role fosters genuine patient interactions and allows for flexible scheduling through our Monday to Friday workweek, typically from 9 AM to 5 PM.
Key Responsibilities
- Conduct comprehensive in-home evaluations to assess patients' physical abilities and needs.
- Design and implement individualized treatment plans based on the specific goals, capabilities, and medical histories of patients.
- Provide interventions including therapeutic exercises, manual therapy, gait training, and balance activities tailored to enhance functional mobility.
- Educate patients and their caregivers on effective home safety measures, fall prevention techniques, and strategies for maintaining overall wellness.
- Continuously monitor patient progress, adjusting treatment plans as necessary to align with evolving functional goals.
- Maintain accurate and timely documentation of each visit in accordance with Medicare requirements and agency standards.
- Collaborate with an interdisciplinary healthcare team, including nursing, occupational therapy (OT), speech-language pathology (SLP), and social work, to provide comprehensive care.
Patient Population / Client Focus
The Home Health Physical Therapist will work primarily with a diverse population of patients recovering from post-surgical procedures, orthopedic injuries, neurological disorders, and other chronic conditions. This role demands a strong understanding of the specific needs and challenges faced by these patients, as therapy is administered in their home settings, allowing for more personalized and effective rehabilitation strategies.
Qualifications
- Active New Jersey Physical Therapist License is mandatory.
- Completion of an accredited Physical Therapy program (Doctor of Physical Therapy - DPT or Master of Physical Therapy - MPT).
- Possession of a valid driver's license, reliable transportation, and car insurance.
- Current CPR/BLS certification.
- Demonstrated clinical judgment capabilities and effective time management skills.
- Ability to work independently within patients' homes, showcasing strong communication skills to interact with clients and families.
Preferred Experience
- Minimum of two years' experience in home health, outpatient rehabilitation, or hospital-based therapy.
- Familiarity with Electronic Medical Record (EMR) systems, particularly HomeCare HomeBase, Kinnser, MatrixCare, or similar platforms.
- Understanding of Medicare documentation guidelines and OASIS assessments preferred, with training provided as necessary.
Compensation and Benefits
The full-time Home Health Physical Therapist will receive a competitive salary ranging from $90,000 to $110,000 per year, commensurate with experience and qualifications. Additional compensation benefits include:
