- Physical Therapist - Travel Contract
Description
Join a home health team in Pittsfield, MA as a Physical Therapist. Provide skilled physical therapy to patients across the lifespan in their homes. Plan, implement, and evaluate individualized plans of care based on referrals and ongoing assessments. Collaborate with the interdisciplinary team to optimize patient function, safety, and independence. Document plans of care, progress notes, and phone orders in accordance with agency and regulatory requirements. Educate patients and families on therapeutic procedures, safe transfers, and appropriate use of durable medical equipment. Order and adapt rehabilitative equipment to meet patient needs and home limitations. Apply infection control and universal precautions. Fulfill assignments consistent with education and competence and perform other related duties as requested.
Minimum Qualifications
- Bachelor of Science in Physical Therapy, Master's, or Doctoral degree in Physical Therapy from a program approved by the American Physical Therapy Association or equivalent accredited program.
- Current state license as a Physical Therapist required to start the assignment in Massachusetts.
- Proof of current CPR certification required.
- Current driver's license, reliable transportation, and vehicle insurance required.
- 1-2 years of experience as a Physical Therapist in acute care or rehabilitation settings required.
- Ability to perform prolonged standing, walking, heavy lifting, bending, pushing, and pulling as required for home health therapy visits.
- Ability to work a flexible schedule and travel locally.
- Must read, write, and speak fluent English.
- Experience with durable medical equipment knowledge and ordering equipment relevant to home care recommended.
- Previous home care experience preferred.
